Professional Disc Jockey and Karaoke Services Contract Agreement
The following is a written agreement between Ed Altounian (THE DJ GUY)
and ________________________________________________________________, hereafter referred to as customer.

THE DJ GUY agrees to provide music services for customer on the ___________ day of ________________, 20______.
The reception/event is to be held at ____________________________________________________________________

Customer agrees to pay The DJ Guy for its services (make checks payable to Ed Altounian).
The balance of this contract is to be paid ten days prior to the event for all weddings unless otherwise instructed.
A minimum $100.00 deposit is required to secure services for any specified date (photo booth requires additional $100 deposit). The deposit is non-
refundable. Cancellations made less than 90 days prior to the event are subject to a cancellation fee equal to 50% of the balance, 60 days 75% and 30
days 100%, less the deposit. Changes to this policy and/or refunds will be at the sole discretion of the THE DJ GUY. Installment payments are limited to
no more than FOUR payments. Neither THE DJ GUY nor any of its associates shall be held liable for any loss or liability as a result of the services
performed. THE DJ GUY will not hold customer responsible for any loss or liability, which may occur to the disc jockey or equipment, unless caused by
blatant or malicious wrong doing by customer or their guests. If the event is to be held outdoors, customer is responsible for providing adequate shelter
from the elements. The DJ assigned to your event is based on availability and package type. Standard wedding packages are excluded from this and
your DJ will be assigned at time of quote, prior to signing this contract. Photo booth attendant and/or DJ reserve the right to end services should guests
become malicious or endanger the well being of any associate of The DJ Guy.
